Most SVS Vision Care plans provide services at any SVS Vision Optical Center. Find a location near you. If your plan includes locations outside of the SVS Vision Optical Center network, check your plan information for a provider near you or call 1-800-225-3095, M-F 8:00 am – 5:00 pm Eastern Time. If you choose to receive services from an out-of-network provider, download an application for benefit reimbursement form.
